Application forms
These are the forms you need to download and complete if you would like to have therapy sessions with us.
If you wish to claim therapy against your insurance policy, you must complete and email or fax the Billing Insurance Registration Form back to us before your first appointment can be scheduled. You can bring the other forms to your first appointment.
The Billing Insurance Registration Form is provided as a Microsoft Word document and as an Adobe Acrobat Document (PDF). You may find it easier to use the Word version because you can enter the details and then email it back to us as an attachment.
If you are going to be paying cash, then just download the Ardent Center Clinical Intake Form and Ardent Center Intake Packet and complete them and bring them to your first appointment. Email us with the general location where you would like to be seen and which therapist you would like to see (see About us for a list of our therapists).
